Does anyone remember the old account admin site?

Before the account admin pages got integrated with the EA one's, there used to be an old site which, when you requested account cancellation, it asked you why you were quitting

You could tick a box, or put in a reason

Anyone remember that?
I was trying to remember the old URL, but can't remember it.

Not sure.
The site people used to go to, was a dark/black themed site, which when you requested account cancellation, it asked you why you were quitting.
