That happened right before I got hacked. I'd suggest changing your passwords for UO and possibly your email (depending on your level of paranoia). In my case, it was my email that was hacked, and the hacker broke into my email and retrieved those username/password reminders (then logged right into my UO accounts like a fresh spring day....)
